Dual Eligible Special Needs Plans (D-SNPs)

Medicare Advantage Plans designed for individuals who are entitled to both Medicare and medical assistance from a state plan under Medicaid.

These types of healthcare plans are offered through an insurance provider (AARP, Humana, Aetna etc.). These include those with low income, with full or partial Medicaid assistance, or disabled but still working. These plans will also come with Part D drug coverage.

Depending on the plan, Dual SNP plans may offer additional benefits such as low copays, no copays, and no out of pocket costs for services.

Dental, Hearing Exams, Hearing Aids, Vision Exams, Eyewear,  Gym Membership, and Fresh Food Boxes.

Healthy Foods
Over the Counter items
Utilities
Transportation -Bus, Taxi, Uber, Gas

 

Chronic Condition Special Needs Plans (C-SNPs)

Medicare Advantage Plans designed for individuals with specific severe or disabling chronic conditions. Some chronic conditions include Chronic Heart Failure, Dementia, Diabetes, Autoimmune Disorders and many others.

Again, these types of healthcare plans are offered through an insurance provider and will come with Part D drug coverage.
